Our vision is for a flagship service to support the shift of higher acuity care from acute hospitals into the community setting. This includes clinical interventions that will prevent hospital admissions and that will facilitate patients being cared for earlier in their recovery following hospital admission in the community setting.



Bury Community Health Team is looking for a Qualified District nurse who displays a passion for the challenge of working with patients in their own homes and local communities.

Successful candidates must demonstrate superior leadership, people and team management skills embracing a commitment to delivering responsive, quality community based services for patients in a multidisciplinary approach to joined up health and social care.

Bury Town Community Healthcare Team is a large team of experienced multi professional staff offering a high standard of nursing and therapy care. The team covers a large geographical area including rural and urban areas, in the heart of East Anglia.

The successful candidate will manage the day to day activity of the team and be able to deal with an ever increasing and complex caseload. They will also monitor, evaluate and improve the efficiency and effectiveness of the service.

- Must be able to travel between patients` homes and base.

- Be able to commit to 8am-8pm working hours including weekend and bank holiday rota.

This is a challenging role but very rewarding for the right candidate

Qualifications/skills/experience:

Level 1 Registered Nurse

Current NMC Registration

District Nurse Professional Qualification

Nurse Prescriber

Extensive experience of working in the community

Experience of student/Staff supervision including Mentorship qualification (ENB998)

Documented evidence of Continuing Professional Development

A high level of ability to reflect on and critically appraise own practice and performance

Excellent communication skills

People management and team skills
