Telehealth Nurse

As a nurse with knowledge and experience in long term conditions, you will be using your expertise to deliver high quality clinical advice to patient’s monitored remotely using Telehealth equipment.

Telehealth is becoming a standard element of nursing care as it allows patients to be monitored daily. Changes in their health and wellbeing are quickly identified and managed, enabling more patients to stay at home. This is an opportunity to be at the forefront of this new way of working.

Within your clinical area you will be responsible for the remote management of patients and the seamless interface of care between Medvivo and the patient’s own specialist team, GP or consultant.

You will provide clinical assessment and triage to patients living with one or more long term conditions by telephone, supported by clinical protocols and aided by decision-support software.

Services are currently provided on weekdays, with no out of hours.

Hours of work will be Monday – Friday, 09.00 – 17.00 (37.5 hours per week)
